NHS Continuing Healthcare (CHC)
NHS Continuing Healthcare (CHC) is a package of care that is fully funded by the NHS for individuals aged 18 or over who have complex, ongoing, or serious health needs. It is designed for those who do not require care in an NHS acute hospital, but whose primary need is for healthcare rather than social care.



NHS Healthcare & CARE4U2DAY – Overview of Funded Services
CARE4U2DAY supports individuals with significant health needs through one CHC NHS-funded pathway:
NHS Continuing Healthcare (CHC)
We are pleased to announce that CARE4U2DAY is working in partnership with our clinical collaborators to apply for registration with the Care Quality Commission (CQC) for the regulated activity: Treatment of Disease, Disorder or Injury (TDDI) FNC Pathway.
Currently, our organisation provides services exclusively under NHS Continuing Healthcare (CHC).
By registering for TDDI, we aim to broaden our service offering to support NHS placement teams and newly qualified nurses, enabling us to deliver a wider range of clinical care interventions.
This registration will allow us to:

Provide nursing care under CQC-regulated standards

Support patients with complex health needs beyond CHC eligibility

Collaborate more effectively with NHS partners and Integrated Care Boards (ICBs)

CHC is a fully funded care provider for individuals whose primary need is health-related. It covers all care costs, including personal care, healthcare, and accommodation.
Eligibility:
Based on a detailed assessment of health needs (complexity, intensity, unpredictability), not diagnosis or finances. Often fast-tracked for palliative or end-of-life care.

Personal care (e.g. washing, dressing, eating)
Service Agreements & Provider Responsibilities
All NHS-funded patients must have a Service Level Agreement (SLA) with CARE4U2DAY, detailing:

Agreed care hours and tasks

Patient needs and preferences
-
Risk and needs assessments
-
Pre-planned care appointment schedules
Important Notes:

NHS funding only covers agreed SLA hours and tasks.

Any additional services (e.g. extra hours, canceled appointments) are chargeable to the patient.

Providers must remain open, honest, and transparent at all times.

Both the company and patient must sign the SLA.
NHS-Funded Nursing Care (FNC) – TDDI
Purpose:
To fund nursing care provided by a registered nurse within you own home.
Eligibility:
-
Within your own home, with a registered nurse to provide nursing care
-
Assessed as not eligible for CHC
-
Requires regular input from a registered nurse
Funding:
Paid directly by the NHS via the local Integrated Care Board (ICB).
Provider Requirements:
Must have access to:

A registered healthcare professional (e.g. nurse, paramedic, doctor)
-
A social worker (for mental health treatment)
-
Or a multi-disciplinary team including the above
